Rates and Fees:

I am an In-network provider for the following insurance companies:

  • Aetna
  • Blue Cross Blue Shield
  • Healthchoice
  • Oklahoma Health Network
  • United Healthcare/ Optum

Documentation can be provided to assist you in seeking reimbursement if I am out-of-network with your insurance.

Helpful Insurance Tips:

You are responsible for verifying your mental health benefits. Services may be covered in full or in part by your health insurance.
The Following questions can be helpful during this verifying process:

  1. Do I have outpatient mental health insurance benefits?
  2. What is my deductible and has it been met?
  3. How does meeting my deductible influence my mental health coverage?
  4. How many sessions per year does my health insurance cover?
  5. What is the coverage amount per therapy session?
  6. Is pre-approval required from my primary care physician?
  7. Is Tiffany Hawkins Berry an in-network provider for my insurance plan?

Private Pay:

If you are uninsured or would prefer your services not be filed with your insurance company, private pay hourly rates are available.

  • For a standard clinical hour (50-55 min) Fees will be disclosed upon request of service.

Sliding Scale:

I offer a sliding scale for cash clients facing financial difficulties. I would be happy to speak with you about what is an affordable option for you.

Initial Consultation: 

We will discuss basic information, including contact information, presenting problem, insurance, and any questions you may have.

What you will need for the first session:

Please arrive 15 to 20 minutes before scheduled appointment time in order for paperwork to be completed. You will need drivers license, Insurance card (if Applicable) and form of payment. Mastercard, Visa, American Express, Discover card, Cash, and Check accepted. Payment is respectfully requested at the beginning of each session.
